Office Policy

Drug Urinalysis
A drug urinalysis will be required as part of your treatment. Follow the link below for more information.
Medical Records
Section 191.227.5 states that there is a fee for paper and electronic records, we have by law 30 days from a written request. You have access to your medical records on the patient portal that you can access at any time.
New Patient Info
First, we must have current medical records of at least the past five years before you will be able to make an appointment.
Let’s talk a little bit about blood thinners. If you are taking blood thinners or supplements, we may ask that you discontinue them prior to your procedure. Never stop these medications without approval from the physician prescribing them.
Here is a short list of blood thinners and the amount of days they should be discontinued prior to your procedure:

If you are taking any supplements such as Fish Oil, Garlic, Ginseng, Vitamin A&E, Ginger, Kava Kava, Clove, White Willow bark or Gingko please stop these 7 days prior to your procedure.
